Crazy Doctor places the player in charge of a clinic where patients arrive with unusual medical problems. Each visitor explains a complaint, but the visible symptoms do not always reveal the complete cause. The player must examine the patient, locate foreign objects or internal problems, and choose a treatment. There is no continuous plot with chapters and a final villain. Instead, the story develops through separate consultations, the clinic’s changing reputation, and the consequences of medical and financial decisions.
Diagnosis begins with observation and the use of X-ray goggles, which reveal objects or conditions hidden inside the patient’s body. After identifying the problem, the player selects instruments and applies them to the relevant area. Some cases have more than one possible solution. However, removing only part of the obstruction or leaving an item inside the patient can create complications and reduce the quality of the treatment.
The game combines medical tasks with decisions about payment. Successful procedures provide gold and reputation, while poor treatment can lead to penalties. When an appointment ends, the player may request a fee or handle the case in another way. Asking for too much can produce a fine or damage trust. Special visitors, including wealthy clients and criminals, offer larger rewards but may also create additional risks.

Crazy Doctor does not use conventional levels based on maps or platforming stages. Its progression is organized around patient cases, diseases, equipment, and clinic improvements. Completing appointments earns the resources needed to unlock new tools and medical conditions. Later cases require a better understanding of the available instruments because similar complaints can have different causes. VIP patients function as higher-value challenges within this structure.
